What identification do I need?
A current, government-issued photo identification that shows your signature — a driver's licence, a state identification card or a passport. The name on it must match the name on the document.
Can you come to a hospital?
Yes, and it is a large part of the work. Tell us the floor, the visiting rules and whether the signer can hold a pen, so the right arrangements are made before arrival.
Do you bring witnesses?
Yes, when the appointment is booked with enough notice. Witnesses cannot be anyone named in the document or standing to gain from it.
Can you tell me which form to sign?
No. Choosing or explaining a document is legal advice, which a notary is not permitted to give. We notarize the document you bring.
