The single 30-day extension
Shows the day 90 date and reminds you that 164.528(c)(1) allows only one extension and the written statement of reasons must reach the individual inside the original 60 days.
Six-year lookback start date
Prints the exact date your log has to reach back to for this request, so you do not pull three years or ten.
Free-or-fee gate
Checks the prior accounting date: the first accounting in any 12-month period must be free, and a later one may carry a reasonable cost-based fee only if you tell the individual first.
Reportable entries after exclusions
Subtracts the treatment, payment and health care operations entries your log carries, because 164.528(a)(1)(i) keeps them out of the accounting.
What late costs at 2026 amounts
Multiplies your open overdue requests by the 2026 reasonable-cause minimum of $1,461 per violation (45 CFR 160.404, effective 2026-01-28).
