One shortcode, [damp-mould-report-form], adds a damp and mould report form with 9 fields: your address and postcode · what is the problem (8 choices: damp or mould, condensation that keeps coming back, a leak, no heating or hot water, gas smell or carbon monoxide alarm, electrical danger, a home that is not secure, something else) · which rooms · does anyone have a higher health risk (5 choices: asthma or weak immune system, a baby, young child or pregnant person, someone aged 65 or over, someone unwell, no one) · when you first noticed it · details · name · phone · email (optional). Seven fields are required, every field has a label, the gas emergency number and your timescales are shown above the form, and every report is listed in WordPress admin, 50 per page.
Why a free contact form falls short: it does not ask which hazard it is, which rooms, or who in the home is at higher health risk, so an emergency can read like a routine repair, and no free WordPress plugin ships an Awaab's Law report form.
Menu names can differ slightly by WordPress version and admin language. Needs PHP 7.4 or newer; WordPress refuses the install on older PHP. No other plugin is required; on Elementor pages, put the shortcode in a Shortcode widget.
