One shortcode, [aoda-accessibility-feedback-form], adds a feedback form with 9 fields: where it happened · your feedback is (complaint, suggestion, compliment, question) · what it is about (8 topics: website or online form, document format, talking with staff, service animal or support person, premises, assistive device, service disruption, something else) · tell us what happened · reply format (large print, accessible electronic document, plain language, Braille, communication support) · how should we reply · name, email and telephone, all optional so people can write in anonymously. Five fields are required, every field has a label, and every submission is listed in WordPress admin, 50 per page. On Elementor or multilingual (for example Polylang) sites, put the shortcode in a Shortcode widget on each language's page.
Why a free contact form falls short: it does not ask where the barrier was, what it concerned or which reply format the person needs, and no free WordPress plugin ships an AODA feedback form, so many businesses post a PDF form that screen-reader users often cannot fill in.
Menu names can differ slightly by WordPress version and admin language. Needs PHP 7.4 or newer; WordPress refuses the install on older PHP. No other plugin is required.
